SAFETY & INTERVENTION F.A.S.T aims to provide a safe, secure environment for all young people participating in our academies, events and workshops. Risk assessments have been undertaken for all activities and all young people will be supervised during the academy, event, workshop. Young people participating in our academy, are considered to be responsible for their own actions. By signing the this form you accept that your child will abide by the rules of the academy; failure to do so may result in the young person being excluded from the Tournament activities for a part of the day. PHOTO PERMISSION We take photos’ and record videos of our sessions. Before taking images of children under the age of 16, we need parental/carer permission. We use images of your child for publicity in our online presence, social media or on our website, reports and funding reports. SPRING TOURNAMENT 2022 Parent/Carer Agreement I /We will drop off and collect my child on the following dates from the venues listed below: April 4th, 5th and 6th. Hillcross Primary School Ashridge Way, Morden SM4 4EE April 7th. Polka Theatre 240 The Broadway, London SW19 1SB April 8th. Tooting and Mitcham Imperial Sports Ground, Bishopsford Rd, Morden SM4 6BF We will authorise any adult person to collect our child by informing the Pastoral Coach of any alternative arrangements. We will provide a contact phone number on the first day of FASTournament. Attendance. My child will attend all of the schedule dates April 4th to 8th. Please note that we’re unable to accept part bookings due to the teambuilding nature of our course. Full Time. My child will attend at the scheduled times: 9:45 a.m. to 3:00 p.m. Please note that we’re unable to accept players attending part day due to the team building nature of each day’s games and exercises.
